1. When you first log in, you will see a page with colorful buttons. These are your forms and folders. Folders are used to group similar related forms.

The blue buttons with 5 lines will take you to a list of completed forms

2. When viewing completed forms, you can use the filter button to find a specific form. Click "Filter"

3. Fill in any piece of information that will help narrow down your options. Click "Filter"

4. To complete a form, click on the button of the form you'd like to complete

5. Fill out as much information as possible. Be aware that some fields may be required prior to submission

6. Click on the "Submit" button at the bottom of page whencompleted. If you would like to save it to complete at a later time, click on "Save as draft"

7. If you saved your form as a draft and want to find it, go to "Draft" on your home page. It is located just above the colorful buttons. You will only see this button if you have Drafts.
