1. Your menu is on the left side of your home screen (this is mobile version). Click "All Forms."

2. Click the "Core Incident Report. "

3. Click the + icon to add a Core Incident Report.

4. To record your voice to fill out the form, click "Start Voice Recording." This will fill in your form for you. You can continue to scroll through the form and speak the information you want filled in.

5. This microphone will appear as your voice inspection is in progress.

6. Fill out the name of the person completing this form (you) and as much information as possible.

7. Fill in information about the employee involved in the incident.

8. Continue to fill in their information.

9. Fill in the information about the physician or other health care professional.

10. If treatment was given at the worksite, fill in this information.

11. Click Yes or No for these questions.

12. Fill in the information about the case.

13. Explain in detail how the incident occurred and the injury or illness.

14. At the bottom of the form you can either save as a draft or submit if completed.
