How do I report changes in my care recipient's care needs?

If you notice any changes in your care recipient's care needs please complete our change in care needs form. Including:

Personal care

Including matters such as personal hygiene, continence management, getting dressed, managing medications, managing skin integrity.

Nutrition or hydration

Including matters such as preparing meals, monitoring intake/output or weight.

Mobilising

Including matters such as walking and transfers, both inside and outside, and any equipment used for this purpose.

Companionship, mental support and wellbeing

Including matters such as providing positive support, managing capacity, communication, mental health, mood or sleeping.

Housekeeping

Including matters such as shopping, laundry, cleaning, tidying or managing finances.

Medical conditions and maintaining health

Including matters such as breathing, controlling temperature, minimising and managing infection, behaviour, managing long-term conditions or recognising deterioration.
