Creating question sets or questionnaires in the forms

Customize your review process by setting up questions and question sets using the following options.

Conversation settings

The 'Ask questions as they are' setting controls on how questions are presented to reviewers:

  • Enabled – questions appear exactly as written in the form.

  • Disabled – questions are reworded to feel more conversational, like a chat; this adapts the wording for self-reviews and other review types.

Configuring question sets

Assign questionnaires to specific review types. You can:

  • Use the same questionnaire for all review types.

  • Create different questionnaires for managers, peers, and other review types (up to four).

'Allow not to fill in' makes the questionnaire optional. If a reviewer opts out, their response is marked as Dismissed.

Question options

Effy AI supports:

  • Open-ended questions

  • Multiple-choice questions with customizable rating scales

Enable 'AI should ask a follow-up question based on the given answer' to gather deeper insights. AI will generate follow-ups tailored to the response.

In 'Ask questions as they are' mode, you can create separate wording for self-reviews when the same questionnaire is used for multiple review types.


Final screen

The final screen text is generated automatically and includes the name of the person who initiated the review.
