The search bar can be located near the top of the ‘Discover’ landing page of the EMGuidance website or app.
Tap into the search bar, and enter the term 'motivation' or 'medical aid' to search.
Select the relevant product listing from the list of returned search results to open and view the content.
In the detail view of the product listing, select the 'Motivation Assistance' option.
Complete the form by entering all the information into the applicable fields.
Submitting the completed Motivation Assistance form to the medical scheme:
Once you have completed the motivation form, you will receive an email with the output letter and supporting documents attached.
Kindly wait 5-10 minutes after completing the form to receive the email.
The output letter will be provided as an editable document which can be updated and placed on a practice letterhead.
Once the above steps are complete, you are now able to submit the motivation form, together with the supporting information, to the applicable medical scheme.
