Overview
The Primary Insured's Information section of the New Claim form captures the policyholder's personal details, policy information, and property characteristics. This section uses a Show/Hide toggle — click the section header to expand or collapse it.
This article walks through every field in the Primary Insured section so you know what to enter and when.
Personal and Contact Information
First Name / Last Name — The insured's legal name. Both fields are required for standard claim intake.
Company — If the insured is a business entity rather than an individual, enter the company name here.
Email — The insured's email address. This is used for automated correspondence such as Acknowledgement letters and Loss Notices when the "Send email" option is selected.
Phone # / Alternate Phone # / Cell Phone # — Up to three phone numbers for reaching the insured. The Cell Phone number is also used if SMS text alerts are enabled for claim communications.
Street Address / Address 2 / City / State / ZIP / Country — The insured's mailing address. The State field is a dropdown. This address can be copied to the Loss Location section using the Copy From Above button if the loss occurred at the insured's home address.
Policy Information
Contract/Treaty — A dropdown for selecting the applicable contract or treaty, if your organization uses treaty-based claim handling.
Policy # — The insurance policy number associated with this claim.
Alt Policy # — An alternate or secondary policy number, if applicable.
Loss Form Written Under (Policy Form) — The specific policy form under which the loss is being filed.
Policy Type — A dropdown with standard policy types (for example, HO3 — Special Form, HO5, DP3, Commercial, etc.). Policy Types are configured by your administrator in Settings.
Policy Section — The section of the policy that applies to this particular loss.
Policy Effective Date / Policy Expiration Date / Policy Renewal — The three key dates that define the policy period. These help verify coverage eligibility at the time of loss.
Date Policy Confirmed — The date that coverage was verified or confirmed for this claim.
Loan # — The mortgage loan number, if applicable. This is relevant when a mortgagee is listed on the policy.
Mortgagee — The name of the mortgage holder or lending institution listed on the policy.
Property Characteristics
Occupancy — A dropdown indicating how the property is occupied (for example, Owner Occupied, Tenant Occupied, Vacant).
Construction Type — A dropdown for the building's construction material (for example, Frame, Masonry, Steel).
Majority Damage Claim — A dropdown to categorize the primary type of damage on this claim.
Claim Flags (Checkboxes)
At the bottom of the Primary Insured section, a row of checkboxes lets you flag special claim conditions. Each checkbox marks the claim for reporting and filtering purposes.
Surplus Line — Check if this policy was written through a surplus lines carrier.
Subrogation Possibility — Check if there is a potential subrogation recovery opportunity on this claim.
Refer to Underwriters — Check if the claim needs to be referred to the underwriting department for review.
Claim Withdrawn — Check if the insured has withdrawn the claim.
Claim Not Paid as Within Excess — Check if the claim falls within the excess (deductible) amount and no payment will be made.
Claim Denied — Check if coverage has been denied for this claim.
