Setting up the Feedback form

In the Feedback form you can edit and compose your Feedback form. To edit your Feedback form, first click on Feedback form.

You can choose the following items: Step 1, Step 2 and Design. All three are explained below. For your information: the field 'by name or anonymous' has been removed from both step 1 and 2.

Step 1

In step 1 you can add a new question to your Feedback form by pressing the blue button as shown in the image below. Here you can also select the type of question and place it on public or not applicable. When a question is not selected on public, the answer to this question will not be published publicly.

Step 2

Step 2 allows you to indicate which personal questions you would like to add to your Feedback form. Check "Show field in form" if you want to include this question in the form. Clicking "Make field optional" will give the reviewer the chance to not answer the question. The image below shows how this looks like. Then click on "Save changes".


When using the branch module, you can enter a personalized label for the branch selection under "Custom label for field". You can use the label for the branch dropdown, which is displayed on your personal review page.

Design

We have added a 'success emoji' to the success page that your customer will see after submitting a review. You have the option to change the emoji and disable the confetti on the success page. The default colors of the confetti are Feedback Company colors. You can customize the confetti to your corporate identity.
