The NY RD SNAP Checklist:
Pre-work:
Identify Food Insecurity via NutriQuiz questions
If they answer “NO” in regards to an EBT card and DO NOT want assistance with SNAP, document in their chart
If they are interested in SNAP signup, schedule patient for a 60 minute follow-up visit
The SNAP Application Appointment:
At the follow up appointment, select SNAP as intervention
Verify that patient has the necessary documentation with them
Verify that patient has a functional, accessible email address
Begin a screen share session with the patient via Zoom
Create and/or document my.ny.gov account with the patient or login to an existing account
Fill out the application with patient
Finish the application and ask for verbal consent to sign off on the application on patient’s behalf
Click “submit” once application is reviewed and signed
Share next steps with patient
Share local SNAP office phone number
Share decision timeline/what to expect
Complete chart documentation for patient

Before connecting with the client
Follow these steps if you have not yet called a client who needs support in another language:
Dial (844) 722-0892
For Spanish, press 1
For Other, press 2
Allow the interpreter to introduce themselves and provide their Interpreter ID.
Introduce yourself to the interpreter.
