The VA RD SNAP Checklist:
Pre-work:
Identify Food Insecurity via NutriQuiz questions
If they answer “NO” in regards to an EBT card and DO NOT want assistance with SNAP, document in their chart
If they are interested in SNAP signup, schedule patient for a 60 minute follow-up visit
The SNAP Application Appointment:
At the follow up appointment, select SNAP as the intervention
Verify that patient has the necessary documentation with them
Verify that patient has a functional, accessible email address
Begin a screen share session with the patient via Zoom
If Audio-Only, inform the patient verbally of each step and read each question thoroughly, verifying the information provided before moving to the next step
Visit the website.
Create and/or document account with the patient or login to an existing account
Fill out the application with patient
Finish the application and ask for verbal consent to sign off on the application on patient’s behalf
Click “submit” once application is reviewed and signed
Share next steps with patient
Share local SNAP office phone number
Share decision timeline/what to expect
Complete chart documentation for patient
Reason for Visit: SNAP Setup
HPI: Pt seen by RD regarding assistance with completion of SNAP application. Pt reports difficulty affording food and screens positive for food insecurity.
Assessment & Plan:
Pt with limited access to food (NB-3.2) related to financial limitations as evidenced by food insecurity screening and discussion w/ patient.
Plan/Intervention: Improve access to food by providing assistance w/ completion of SNAP application.
Monitoring and Evaluation: Approval of SNAP application and amount awarded; changes in food security.
Diagnosis: Use “Diet Education” for diagnosis, diagnosis code is Z713
Provided assistance in completion of SNAP application. Application submitted successfully. Discussed next steps including interview w/ a case worker and timeline for benefits approval.
Patient Instructions:
It can take up to 30-45 days to get your SNAP benefits, or up to 90 days if a disability decision is required. If you qualify for emergency service, then you may get SNAP benefits sooner.
If you have to wait longer than the number of days we say it should take to process your application, you can call (804) 726-7361 to check on the status of your application.

Billing:
ALL SNAP enrollment visits are considered follow ups
Billing code remains the same as a regular follow up visit - refer to the RD Guide by Plan/Pilot
Diagnosis code is Z713 as usual
Reminder: If RD schedule allows, please spend the time to fully complete the application.
If RD schedule does not allow for continuing past the 60-min mark and the application is incomplete, schedule another 30-min SNAP visit to complete the application.

You may be exempt from the work requirement if you are currently working or participating in an approved work program; responsible for the care of a child; pregnant; medically certified as unable to work; meet one of several work registration exemption reasons; or live in an exempt locality.

Social Security Number for all applicants

Information about resources/assets

*Medical expenses for elderly or disabled household members

*Deductions: expenses that may make you eligible for more SNAP benefits

After submitting your application, an interview will be required unless you meet the conditions for a waiver
A face-to-face interview is required unless the local agency agrees to conduct the interview by telephone or a prearranged home visit. Waiver of the face-to-face interview is determined on a case by case basis depending on the household’s circumstances that include but are not limited to: illness, prolonged severe weather, transportation difficulties, or work hours during normal agency office hours.
Proof of identity (driver’s license or picture ID), residence, income, resource and shelter expenses will be required
