Introduce the Diabetes Plate Method.
Metrics: all recent labs (A1C must be included), symptoms (neuropathy, frequent urination, frequent thirst, fatigue), and weight. A1C to be checked every 3 months (at the end of 12 weeks if seeing the patient weekly, 6 weeks if seeing the patient bi-weekly). Encourage monitoring of blood sugars if able (pre & post meals).
