As part of our partnership, AAH employees are provided with several wellness benefits through which they can complete their Healthy Living Activities, a health incentive program. Foodsmart is one of those options. For more thorough instructions regarding program requirements, click the link above.
Type: Employee Sponsored Program
Program Goal:
To engage their employees in Healthier Living practices, specifically for our program, weight loss and healthier at every size.
2025 Program Requirements:
Members must register for an account through foodsmart.com. They can sign in to their prior account and make sure to proceed through the registration button or create a new account if a new user. (THIS STEP MUST BE COMPLETE BEFORE THE END OF THEIR FIRST VISIT)
Members schedule their RD appointment
Members must complete:
FOUR RD visits (Four are required, and no more than 4 may be completed this year!)
Pick a program for 2025: Healthy Weight, Gut Health, Money Saving, Eating for Fitness (The program they choose MUST be documented at visit 1)
Complete a NQ prior to visit 2 and at visit 4 (it is recommended that the RD complete the NQ with the member on the phone during visit 1 and 4) Completing a 2nd NQ mid-program does NOT complete their requirements, it must be at the beginning and end of their engagement.
Any assigned readings/homework assigned by their RD as part of their curriculum between visits.
A certificate of completion will be sent after the program is completed.
Enrolling Patients into Programs:
Pick a program for 2025: Healthy Weight, Gut Health, Money Saving, Eating for Fitness (The program they choose MUST be documented at visit 1)
Document the date, selected program, and topic discussed in the patients sticky note, HPI, or Assessment and Plan sections (under Intervention is a typical location).

Eligibility: Employees and Dependents
Telenutrition Requirements:
To get started with the program, complete your first Telenutrition Visit.
To access your Telenutrition appointment, follow the instructions in your confirmation email.
During your first Telenutrition Visit, you will select your desired educational curriculum.
Curriculum choices include Healthy Weight Loss, Nutrition for Fitness, Gut Health, or Healthy Eating on a Budget.
Complete three (3) additional Telenutrition visits three to five weeks apart within a 16 consecutive week period.
You will complete a total of four (4) visits throughout your engagement.
Your 16 consecutive weeks of engagement begins when you complete your first Telenutrition Visit.
Once engagement has begun, it cannot be paused or restarted.
Complete all assigned curriculum work between your four (4) Telenutrition visits.
Complete all above requirements by Nov. 21, 2025
You must fully complete your program by Nov 21, 2025.
If you screened with a BMI ≥ 30, complete an onsite Follow-Up Screening and show one of the following: 5% weight loss, BMI < 30, or a body fat percentage within the healthy range for your age and gender to earn the remaining $150 portion of your Healthy Living Reward. You must complete your Follow-Up Screening onsite by Nov. 21, 2025.

Congratulations on selecting Foodsmart as your Healthy Living Support Program and taking important steps toward improving your health! The registration and program requirements below must be completed by the applicable deadlines in order to be eligible for the $250 portion of your Healthy Living Reward.

Registration Requirements:

Select Schedule Appointment
Click Get Started and follow the prompts to schedule your initial visit.
After you schedule your appointment, you will receive a confirmation email with instructions on how to access your appointment and tips on how to prepare.
Returning Users:
Log into your existing Foodsmart account that you created in 2023.
A pop-up box will appear welcoming you to the 2024 Foodsmart program and asking you to click to proceed. Click on the button in the pop-up box to create your 2024 Foodsmart Account.
Schedule your first Telenutrition Visit.
To Schedule, select the Telenutrition button at the bottom of the home screen.
Select Schedule Appointment
Click Get Started and follow the prompts to schedule your initial visit.
After you schedule your appointment, you will receive a confirmation email with instructions on how to access your appointment and tips on how to prepare.
You must activate your 2025 Foodsmart account AND schedule your first Telenutrition Visit by July 25, 2025. Your registration is not complete until your first Telenutrition visit is scheduled.
The Healthy Living Team
*Advocate Health covers the $99.99 cost of your Foodsmart account. This will appear one-time as non-cash income on your paycheck (coded on your pay stub as "MISC INC"). The taxes will be applied to your paycheck the month after your registration is complete. If a spouse/partner is enrolled in the membership, the taxes will be applied to the medical plan holder's paycheck.
