Credentialing FAQs

Q: What is the difference between credentialing and enrollment?

A: Credentialing is the process of verifying your education, licensure, certification, work history and other supporting documents to present to a Credentialing Committee to make a decision for participation. Enrollment is the submission of paperwork to a government entity or Payer.

Q: What are the different types of enrollment?

A: Government enrollment is submission of an application for participation in a Medicaid or Medicare program to be issued an ID number to treat patients. Payer enrollment is submission of an application to an individual Payer to participate in their network.

Q: What is the order I complete my certification, licensure, credentialing, and enrollment?

A: You must first obtain your Commission on Dietetic Registration (CDR) certification, then licensure in your home state and any state(s) where you initially intend to provide patient care, if a license is offered in those states. Then, your credentialing with Foodsmart must be approved by a Medical Director or Credentialing Committee. After that, Medicaid enrollment(s), Payer enrollment(s) and additional licensing is completed in the required states where you intend to provide care.

Q: What states do not have a licensing program for Dietitians?

A: Arizona, California, Colorado, Michigan, New Jersey, and Virginia do not currently offer a Dietitian or Nutritionist license or registration. Michigan and New Jersey are currently pending legislation for formal licensure. All other states offer a state license or registration with a Dietitian or Nutritionist credential.

Q: What is required for licensure or certification in order to contract with Foodsmart?

A: A current CDR is required for all states, and a license is required in your primary state of residence (if offered by the state), as well as the state where patients are seen (if offered by the state).

Q: What states require Medicaid enrollment?

A: Foodsmart is actively working towards enrollment in all states. Refer to WorkRamp trainings for the most current enrollment initiatives.

Q: What states require Payer enrollment?

A: At this time, Oregon, New York, Illinois all have Payers that require separate enrollment/credentialing.

Q: Do I have to have a CAQH profile?

A: Yes, a CAQH profile is required for Payers that need the CAQH application for credentialing in their individual network. The CAQH Provider User Guide can be accessed here.

Q: How can I get insurance coverage for my credentialing? What kind do I need to get?

A: Employed providers and contractors (as of 2/9/26) are covered under the Foodsmart professional liability insurance policy.

Q: How often am I required to re-enroll with Medicaid?

A: Most state Medicaid programs require revalidation approximately every 5 years. The program should notify Foodsmart prior to the revalidation period.

Q: How often am I required to re-credential with Foodsmart or a contracted Payer?

A: Recredentialing is required at least every 3 years, but may be more often, depending on the credentialing decision. Foodsmart and Payers will notify you prior to your recredentialing due date, which should also be noted on your current credentialing approval letter.

Q: Do I need to update any information after my credentialing or enrollment has been approved?

A: Yes - You will need to update your CAQH profile, Medallion and all state Medicaid programs that you are enrolled with the following information: License renewals, CDR renewal, professional liability insurance renewals (CAQH and Medallion only), and demographic information updates (addresses, name changes, etc). Re-attestation in your CAQH and Medallion profile is also required every 120 days. Learn how to re-attest in CAQH in their Provider User Guide here.

All state Medicaid programs require practitioners to submit copies of their renewed license/s or certification/s to maintain active enrollment status. Failure to submit the renewals prior to their expiration will result in the state terminating your enrollment and you would need to re-apply for enrollment from the beginning.

Terminated enrollments will cause RD appointment scheduling issues. Foodsmart is required to confirm the submission to the state and may require approval prior to resuming patient care in that state.
