The most important piece of information to include when reporting a bug is the Skill Code.  This can be found in many places.  In the teacher app, you will find it any place that you can view a sample problem (from student's current problem, from the Spiral Review report, or from the WORK screen).

In the student app, it can be found under the selected problem or from Statistics in the student menu.

Choose Statistics.
