Here are the instructions on how to copy a form:

1. On the main page, go to whichever campaign your form is in, and then click on "Form List." 

2. This will bring you to the full list of forms linked to the specific campaign. Navigate to the form you want to copy, and click on the corresponding "Copy" button located between "Preview" and "Move."

3. Update the Internal Name to reflect the new form, update the hosted page to reflect the new form, and update the default label to reflect the new form. This is to ensure the form has unique identifiers.

*Note: If you have information listed in the header that is related to the old form, you may need to review that information and update as needed for the new form you are creating.

Additionally, you will need to go to the "Confirmation Messages" tab for the copied form, and update this messaging to reflect the new form. Go through each section within this tab, and make sure the language reflects the new form.
