Designing a custom survey is quick and easy. Before beginning, you will want to gather some information about the survey. This information includes:

Setting up the Survey:
To set up the survey, follow these instructions:

After Creating a Survey, you will be taken to the design survey page. You can also access the survey designer page by going to Communication > Surveys. Click the action dropdown for the survey you need to design and click "Edit."
Manage the banner. You can remove the banner, upload a custom one, or revert it to the default banner.
Open/Close Survey. You can open and close the survey. To set it to open and close automatically, visit the Survey Settings.
Create a survey description. Optionally, click "Add description" to add a survey description.
Select a color scheme using the "Font Color" color code picker. The color scheme is used for the area below the banner, certain text, buttons, and other areas. You can enter RGB, HSL, or HEX color codes to find a specific color (as shown below).
Click "Save" to ensure all the changes are made.
Creating Survey Questions:
After establishing your survey, you will want to enter the survey questions. Follow these directions to enter survey questions:
Open the Survey editor page.
You should already have one question started, but if it is not, click "Add New Question."
Create Questions. Create the first question using the following steps:
Enter the question text.
Select the question type.
Free text: Members can enter any text.
Multiple Choice: Members can select only one answer.
Multiple Answers: Members can choose multiple answers.
If selecting multiple-choice or multiple answers, enter the possible answers. To enter more possible answers, click "Add Option."
Select optional question conditions (Note: The options are discussed in greater detail below.):
Enable Other Responses (Multiple Choice/Answer Questions): You can add a single-line answer if the available answers are limited.
Conditional Questions: A conditional question will only be shown if a specific answer is selected for a preceding question.
Require this question to be answered: Make the question mandatory.
Click the green "Save," and the first question will be saved.
To enter additional questions, click "Add New Question" and repeat the previous step.
You can click and drag the questions to change the sort order using the hamburger icon (if necessary).
Click "Save" (on the bottom right) to save the survey form. The survey will then be created.
Free Text Option for Multiple Choice/Answer Questions:
Occasionally, in surveys, we ask a question that asks the respondent what their favorite item is. For example, we might ask, "What is your favorite golf ball color?" Three primary colors are used for golf balls, but several brands have other colors (e.g., red, pink, two-tone, patterned, etc.) However, we may not want to include all possible answers. Otherwise, the question might get a bit long.
Therefore, when creating a Multiple-Choice or Multiple-Answer Question, we can include an option for "Other" responses using free text. This allows the respondent to include their own answer. To include the Free Text option with a Multiple-Choice/Answer question, check the box for "Enable Other Response" and specify the option text (if necessary) (as shown below).

Creating Required Questions:
When creating a survey, you may want to require some questions to be answered. Therefore, you can set a question as required to be answered. To make a question required, check the box "Require this question to be answered" (as shown below). The same action can be taken on several questions.
Note: We understand that making all the questions mandatory in your survey may seem necessary, but this can lead to frustration and a higher chance of respondents abandoning the survey. We recommend limiting the number of required questions to ensure a higher response rate and greater accuracy of responses.
Creating Conditional Questions:
When creating a survey, some questions may only be relevant based on a previous answer. Therefore, you can create conditional questions if a particular logic is met. When a question is set as conditional, it will only be shown to the member if the defined logic is met.
To create conditional question logic, follow these directions:
When creating a question, check the box for "Conditional Question."
Define which question determines if this question should be shown using the dropdown.
Should the question be shown if:
Include: Include the question if the member picks the selected answers.
Exclude: Exclude the question if the member picks the selected answers.
Check the box for each answer that should be used to meet the logic in the prior step.
Click "Save."
If the same logic should be applied to another question, click "Apply rule for other questions." Select the additional questions to which the logic should be applied (as shown below). The selected question must meet the logic to allow members to answer these questions.
Additional Notes regarding conditional logic questions:
Conditional Question logic cannot be applied to the first question in your survey.
Only a question that precedes a conditional question can be used to determine if that conditional question is displayed.
A Conditional Question cannot determine if another conditional question is shown.
Editing a Question:
To edit a question, hover your mouse over the question and click "Edit" (as shown below).
Deleting a Question:
To delete a question, hover your mouse over the question and click "Delete" (as shown below).
Deleting an Answer:
When editing a question, click the red trashcan next to the answer you wish to remove (as shown below) to delete an answer.
Reordering Survey Questions:
Occasionally, you may need to reorder your survey questions if they are out of order. To reorder your survey question, click on the three gray bars, drag the question to its proper location, and release (as shown below).

Occasionally, you may need to reorder the answers to your survey questions if you feel they are out of order. To reorder your survey question answers, when editing a question, click on the three gray bars, drag the answer to its proper location, and release (as shown below).
