The risk is a score made up of the severity of harm and the likelihood that it would occur.

Using the HANDS HQ standard 5x5 matrix if the severity of harm was a broken bone and it was very likely it would happen the risk would be 20 (4 major x 5 very likely)

You would then implement control measures to decrease the likelihood of harm and the residual risk score.
