SITE INFORMATION
Site Address and QR Code: When you receive your Hazard Board, the full site address will already be filled in and the QR code attached to the Hazard Board.
Site Supervisor: Name of the site supervisor
Site Supervisor Phone Number: Contact number that is available 24 hours a day (e.g. an office line and/ or a mobile number).
EMERGENCY INFORMATION
Emergency Contact / First Aider: The name of the trained First Aider/s.
Emergency Contact / First Aider Number: The contact number of the trained First Aider/s.
Site First Aid Kit Location: The location of first aid equipment e.g. First Aid kit. Could be a physical location within the site premises, or a supervisor’s vehicle parked out front etc. Display a first aid sign or label to help workers easily find the location of the kit.
Site Fire Fighting Equipment Location: Location of where to locate fire equipment e.g. fire extinguisher. Display a suitable sign or label to help workers easily find the location of the firefighting equipment.
Site Emergency Signal: In the event of an emergency, how will the emergency be announced? Consider how you can raise the alarm so that all workers spread throughout the site can reasonably hear it e.g. air horn, shouting ‘Emergency, Emergency, Emergency’ etc.
Assembly Area: Where workers are to assemble if they need to evacuate the site. Provide the address and, where possible explanation. e.g. Across the road at 123 Smith Street.
CURRENT SITE HAZARDS
Tick off the hazards that are present on-site.
Tick what PPE must be worn on site.
There is also some extra space to add a couple more hazards and PPE that may not already be listed.
FILL IN AND REVIEW
Hazard boards come with information on the board provided in SSSP.
Affix the Hazard Board so it is clearly visible at the front of the site, ensuring both workers and members of the public can view it.
Update the date that the Hazard Board was last reviewed, to ensure it is regularly checked and remains current.
Regularly check that the Hazard Board is legible, meaning that it can be read by workers and members of the public. For example, after strong weather conditions, check that the Hazard Board has not been damaged or contact details have not worn off.
