Health Risk Assessments (HRAs) are optional health surveys that an agent can fill out on behalf of their client after the enrollment is submitted. And some carriers pay agent bonuses for completed HRAs.

Accessing HRA forms
After submitting an integrated enrollment, HRA forms can be accessed from the last page of the enrollment flow – the Application Submitted page – or from the Contact’s Profile page inside your HealthSherpa Medicare account.
The Application Submitted page: After successfully submitting an enrollment, you'll land on this page. Click the Complete Health Risk Assessment button to launch the HRA form:
The Contact’s Profile page: After submitting an enrollment, on the Contact's Profile page, you can access the HRA form by selecting the HRA button:
Some HRA forms will keep you in the enrollment flow, others will direct you to the Carrier's hosted HRA form.
