A Health Risk Assessment (HRA) collects health-related information about a consumer

Health Risk Assessments (HRAs) are optional forms that an agent can fill out on behalf of their client after the enrollment is submitted. An HRA collects health-related information about a consumer.

Here is a sample of what an HRA form could include:

Some carriers allow agents to complete HRA forms directly from their HealthSherpa for Medicare account. These carriers include:

  • Anthem (all Elevance brands, including Simply, Wellpoint, Healthkeepers)

  • CareSource

  • Cigna

  • Devoted Health

  • Gold Kidney

  • Humana

  • Medical Mutual

  • Molina

  • Priority Health

  • Quartz

  • Select Health

  • United Healthcare

Note: Some carriers offer agents bonuses for completing HRA forms. HealthSherpa for Medicare is not involved in HRA bonus payments. Please reach out to the respective carriers directly should you have any questions related to HRA bonuses.


Accessing HRA forms with your account

After submitting an integrated enrollment, HRA forms may be accessed from the consumer’s application submission confirmation page as well as from the Contact’s Details page.

  • The consumer’s application submission confirmation page: HRA forms may be accessed from the consumer’s application submission confirmation page.

    1. After successfully submitting an integrated enrollment, you will receive an application submission confirmation page:

    2. From this page, select the ‘Complete Health Risk Assessment’ button to launch the HRA form

  • The Contact’s Details page: HRA forms may be accessed from the Contact’s Details page within your HealthSherpa for Agents account.

    1. After successfully submitting an integrated enrollment, an ‘Enrollments’ section will appear on the Contact’s Details page:

    2. Here you can access the HRA form by selecting the ‘Open HRA’ button:

While some HRA forms may be completed within HealthSherpa for Medicare, you may be taken to the respective carrier’s site to complete others.


Time limits for completing HRA forms

Carriers can impose time limits for completing HRA forms. Time limits are often about three days following application submission. If you attempt to access an HRA after the time limit set by the carrier, you may encounter an error message.
