Open referral links

Isla enables staff to manage all patient referrals centrally from a single, unified interface. The system supports e-RS referrals alongside secondary and tertiary care provider referrals, streamlining triaging workflows across both individual services and Single Point of Access (SPoA) teams.

Instead of manual referrals by email or phone, Isla offers a standardised referral form and attachment submission process, which is fully customisable per site and clinical application. This link is accessible from any web browser to staff, who must sign in via NHSmail or a clinical email address.

Incoming referrals will then populate dedicated worklists on Isla for efficient review, triaging, and processing by clinicians and administrators. The referrer will receive confirmation of their referral submission and any response by the triaging staff member, reducing duplicate referrals and administrative burden.

Triage staff can then immediately place patients onto automated care pathways, speeding up time-to-treatment.

Setting Up a Referral Link and Worklist

1) Request for a nodes pathway to be built in Isla on the relevant team (see below for an example). To request for a pathway to be built, please reach out to your trust programme manager (TPM) or submit a request via the BAU form.

2) Navigate to your Isla team page and create a new referral link. Teams can host multiple links tailored to specific clinical indications or pathways. Please note that referral configurations can only be created by team admins - please reach out to your team admin for this, or contact your organisation admin if you should have admin status.

3) Define the link name, the onwards patient pathway, the required clinical form, whether media attachments (e.g. photos) are mandatory, and preset clinical centres/organisations (referrers can also type custom centres if unlisted).

4) Create a worklist for incoming referrals requiring review, filtering by the outcome node in the pathway. Please note that worklists can only be created by team admins - please reach out to your team admin for this, or contact your organisation admin if you should have admin status.

5. The new worklist will now be visible on your homepage.

6. Your referral link is now live and accessible from any browser. It can be copied and shared directly from the team page.

Submitting a Referral

1) Open the referral link in any web browser and log in using NHSmail or a staff email address.

2) Fill out the required referrer contact and service information.

3) Enter the patient's NHS number to automatically pull their name and date of birth from the NHS Personal Demographics Service (PDS). If the record is not found, enter the details manually.

4) Populate the custom, structured referral form.

5) Attach required photos or documentation.

6) Review and submit the form. An automated confirmation email will be sent to the referrer.

Reviewing your referrals

1) View submitted referrals on your Isla worklist, organised by pathway.

2) Open a referral to view the structured form and any media attachments directly on the patient’s record.

3) Click Record outcome on the patient's record, select the appropriate outcome option, and add comments.

4) Once actioned, the referral is automatically removed from the worklist and integrated into the team’s standard Isla caseload.
