Collecting Custom Information from Families at Registration

How to add custom fields to your registration form to collect information from families — text, dropdowns, checkboxes, and file uploads.

Every family and program is different. Homeroom lets you add custom fields to your registration form to capture exactly the information you need.

Types of custom fields

  • Free text — allergies, emergency contact name,medical notes, special instructions

  • Single choice/Multiple choice — T-shirt size, skill level

  • Checkbox — photo consent, policy agreement

  • File upload — doctor's note, signed waiver

Adding custom fields

There are a few places where you can add registration questions.

  • Under Setup - General info - These are account level/global questions and can apply to all seasons.

  • Season - Registration Question tab - These can apply to only the activities in that season.

  • Activity - When editing an activity you can activity specific questions.

  1. Go to the section where you want to configure the question and open the program you want to configure

  2. Click the Edit Icon

  3. Click Add Question and choose your field type

  4. Label the field and mark it as required and/or to ask at checkout

  5. Save

The questions appear in the order they appear. They can not be re-ordered at this time.

Viewing submitted responses

Custom field responses are stored per-participant and accessible from the student's enrollment record and on enrollments repots.

💡 Tip: Custom fields can be per-program, so you can ask different questions for different programs without affecting others.
