To create a Rating question type:
Select Rating from the Drop Down Question Type
Type your question
Select your preferred Attributes
Select the Score Weight if you have enabled Scoring Options
Set the score bar to your accepted rating score
Click on Preview to review your question
Click on Save to save and create your question
