Recording patient vitals and measurements in encounters
Updated this week

When you record encounter notes, in the Examination area, you can record the patient's vitals and measurements without having to open the Patient Data area of the chart.

📌 Note: When you sign the encounter, the vitals and measurements are added to the Patient Data area of the patient's chart. For children, the CHR automatically updates their growth chart in the patient dashboard section of their chart.

Steps

1. Start an encounter for the patient. See Starting an encounter.

2. At the top of the Examination section, click Add Vitals. The vitals and measurements entry fields display in a table format.

💡 Tip:

3. In one or more of the entry fields, type the patient's values. For example, you can enter height, weight, systolic and diastolic blood pressure, heart rate, respiratory rate, temperature and O2 saturation.

💡 Tips:

4. To add multiple values for the same measurement, click + Add Vitals again and type the values in the new entry line.
