Using Qnaires to update patient data

You can configure your Qnaires to push patient responses automatically to the patient data section of their chart.

Certain Qnaire variables map to a specific data point in the CHR. The mapped data moves from the patient’s Qnaire responses to their chart. For example, a patient completes an intake Qnaire that gathers demographic information. This information automatically populates the patient's demographics section in their chart.

When patients complete a Qnaire with responses linked to patient data templates in their chart, the default CHR setting requires that you must confirm, edit or reject the updates before you can see the changes in the patient’s chart.

You can configure linking Qnaire responses (response options) to patient data in most question types, except picture, draw, history and medication type questions. You can link a text question-type response directly to patient data.

📌 Note: Certain patient data points are pre-configured, such as demographics, social history, and vitals. If you want to collect specific patient responses from Qnaires and link them to patient charts, contact TELUS CHR support (see Contact us) to add patient data templates.

Steps

1. From the Questions page within an existing Qnaire, scroll to a text question type or the questions' response option where you want the patient response to pull into the patient chart.

See Creating Qnaires for information on how to navigate to the Questions page.

2. Click to expand the field.

3. Scroll to the bottom of the text question or response option, and select Update Patient Data.

📌 Note: To edit the pre-configured patient data settings, click Edit.

The Saving Patient Data Setting window opens.

4. Choose a Category, Property and Field from the three lists.

📌 Note: You must make your selections from left to right because the options are linked to each other.

Depending on the selected patient data category, property and field, new fields such as When the checkbox is checked and When the checkbox is unchecked could appear.

5. Perform one of the following actions:

  • If you want the patient data point in the chart updated when the response option is chosen, select When The Choice is selected and select an option from the ones that appear.

  • If you want the patient data point in the chart updated when the response option is not chosen, select When The Choice is not selected and select an option from the ones that appear.

    📌 Note: The available options depend on the patient data category, property and field selected in step 4.

6. Click Save.
