To use TELUS Collaborative Health Record (CHR) Intake on your tablet and/or cell phone, you must first download and install the app to your device using the following links:

Alternatively, you can search for and download TELUS CHR Intake from the Apple App Store (iPhone and iPad users) or Google Play (Android users).
📌 Note: The minimum OS versions for iOS devices is iOS 9.0 and for Android devices is 4.1 Jelly Bean.
