You may have a long list of Qnaires in your CHR account, and may not use most of them on a regular basis. You can group common Qnaires that you use into folders, to avoid scrolling through a long list of clinic Qnaires every time you want to send one to a patient.

For example, if you work in a multi-specialty clinic with many Qnaires that do not apply to your field, you can create a folder with only the Qnaires that you commonly use. Or, if you work in a clinic with many Qnaires per condition/group (e.g. Intake Qnaires), you can create folders for each group. Having relevant Qnaires organized in folders makes it easier to find and access them.

📌 Note: You can create folders and add Qnaires and letter templates, if you have the Folders permission. To add encounter and form templates to folders you must have the Folders and Template: Encounters and Template: Forms permissions. See User permissions.

Steps

1. To create your own folders, click your avatar and click Contents. The My Contents window opens.
💡 Tip: Any existing folders appear in the My Contents window.

2. To create folders for other users, from the main menu, click Settings > Folders, search for and select a name.
💡 Tip: To search for a name, use the Google Chrome search function (Ctrl or {command}+F) to open your web browser's search functionality.

3. If you already have one or more folders, those will appear in the window that opens. Otherwise, it will be empty. Click +Add Folder.

The Folder Content window opens.

4. In the Name text box, add the folder's name, for example, Intake Qnaires.

5. In the Description text box, type any additional information about the folder.

6. Click +Add Content. A tabbed menu opens.

7. From the Qnaires tab, search and select a Qnaire to add to the folder. When you are done adding items, click away from the tabbed menu.

💡 Tip: To search for a Qnaire, use the Google Chrome search function (Ctrl or {command}+F) to open your web browser's search functionality.

📌 Note: You can add encounter, form, and letter templates to your folder. See Organize your templates into folders.


8. To remove a Qnaire from your folder, hover over the name and click the x that appears to the right.

9. Click Apply.

10. To remove the folder, click the trash icon. A confirmation window appears. Click Okay if you are sure you want to delete it.

11. To add more items to the folder, select it and follow steps 6 to 9 above.

12. Click Close.

Now, when you choose a Qnaire, the Select Folder window opens, you can either select a folder or All Questionnaires to find the applicable Qnaire. See Sending Qnaires to patients from their patient chart
