Configuring transitions for your Pathways

You can use Transitions to customize what happens when you move an appointment from one phase to another. Once built, you see all custom transitions in the same area as the default transitions.

πŸ’‘ Tip: The default virtual visit transitions are:

  • Cancel Visit

  • Complete Visit

  • To Post Visit

  • To Pre Visit

  • To Visit

You can use Validations to add required checks to your transitions. If you have not completed all required tasks when you transition a pathway card to a different phase, a validation warning appears. You must complete or remove all actions before you can transition.

Post Actions are actions that take place when you use a transition, such as staff or patient tasks. They can also be automatic actions, such as changing the appointment status or sending a Qnaire.

πŸ“Œ Note: You must pre-configure the tags, patient actions, staff actions, rooms, message templates and Qnaires that you want to use.

Steps

  1. From the main menu, click Settings > Pathways > Transitions.

  2. Click Add Transition, or to edit an existing transition click the transition from the list.

    πŸ’‘ Tip: To delete a transition, click the x.

  3. Type a Name and choose Colour.

  4. Select whether to enable a Confirmation. If enabled (Yes), you must add a Confirmation Message and the user must agree for the transition to take place.

  5. (Optional) To add a validation, click Add Validation, or to edit an existing validations click it from the list. The Manage Validation window opens. From the list, choose the Target, Type and Condition. Use the following table as a reference. When you configure a validation, a warning appears when you use that transition and the set targets are not completed.

Note: Does not apply if the pathway card is a virtual visit.

  • Should be equal to: Validates that a particular room is assigned. From the Resource list, choose the room.

  • Should not be equal to: Validates that a particular room is NOT assigned. From the Resource list, choose the room.

  • Should be present: Validates that any room is assigned.

  • Should be blank: Validates that no room is assigned.

Assigned Tags

  • Should include: Validates that a particular tag is assigned. From the Resource list, choose a tag.

  • Should exclude: Validates that a particular tag is NOT assigned. From the Resource list, choose the tag.

  • Should be present: Validates that any tag is assigned.

  • Should be blank: Validates that no tags are assigned.

Assigned Staff Actions

  • Should be completed: Validates that a specific set of actions is completed. Use the Condition field to indicate which patient action must be complete.

    • All

    • Important (the Important condition is set in the Staff Actions Pathways settings).

    • Specific (from the Resource list, choose the staff action)

  • Should include: Validates that certain actions are included. From the Resource list, choose the staff action.

  • Should exclude: Validates that certain actions are NOT included. From the Resource list, choose the staff action.

  • Should be present: Validates that any action is included.

  • Should be blank: Validates that no actions are included.

Assigned Patient Actions

  • Should be completed: Validates that a specific set of patient actions is completed. Use the Condition field to indicate which patient action must be complete.

    • All

    • Important (the Important condition is set in the Patient Actions Pathways settings)

    • Specific (from the Resource list, choose the patient action)

  • Should include: Validates that specific actions are included. From the Resource list, choose the patient action.

  • Should exclude: Validates that specific actions are NOT included. From the Resource list, choose the patient action.

  • Should be present: Validates that any action is included.

  • Should be blank: Validates that no actions are included.

6. (Optional) To add an automatic post action, click Add Post Action, or to edit an existing Post Action, click it from the list. The Manage Post Action window opens. From the list, choose the Type and any additional configuration. Use the following table as a reference.

Change status of an appointment

πŸ“Œ Note: Only applies for pathway cards created from an appointment.

Automatically changes the appointment status when the card is moved to the next phase. Select the Appointment Status from the list.

Assign to a room

πŸ“Œ Note: Does not apply if the pathway card is a virtual visit.

Assigns the appointment to a specific room when the card is moved to the next phase. Select a Room from the list.

Assign tags

Assigns tags to the appointment. Select a Tag from the list.

Assign patient actions

Assigns patient actions to the appointment card. Select the Patient Actions from the list and choose whether the actions are Required.

Assign staff actions

Assigns staff actions to the appointment card. Select the Staff Actions from the list and choose whether the actions are Required.

Send Qnaires

Sends the selected Qnaires to the patient. Select the Qnaires from the list, select the Inbox User who should receive the Qnaire responses, and select the Primary Sending Method.

Send a message

Sends a message to the patient using the selected Message Template. Choose the sending user from the list. To create a new message template, see Creating message templates.

Complete current visit

Completes the current visit and removes it from Pathways.

Create encounter

Automatically creates an encounter. For example, the move from pre-visit to visit automatically creates an encounter.

7. Click Save when you are finished configuring the Transition.

πŸ“Œ Note: To set your default phase, click the phase icon to the right of the transition. This sets the default for any virtual visit appointments, when that appointment type is not configured for pathways via an auto assigned workflow phase. When the patient signs into the virtual visit, the appointment will automatically move through this default transition and phase. For more information, see Creating and modifying appointment types.
