Connect to the Io-base mobile application.

At the bottom of the screen, tap on the Forms menu.

To access a form, you have to scan a QR Code.

Prerequisite : you must allow the use of your phone's camera

Select the camera that you want to use for the QR code scan.

Place your QR Code in front of the camera.

Once the QR Code is scanned, the metric(s) associated with the form appear.
