To add your Forma Benefits Card to your Amazon account, please follow the steps listed below:
Log into your Amazon Account.
Go to "Your Account" and then "Your Payments".
Click on "Add a payment method".
Click on "Add a credit or debit card".
Enter your card info, and don't forget to click on "Add your card".
