Kalderos will be contacting, by email, the Authorizing Official and/or the Contact Person listed in the Office of Pharmacy Affairs (OPA) database. It is very important and a requirement of 340B program participation to maintain the accuracy of the OPA database. Any changes, including the updating of the Authorizing Official and/or the Contact Person should be addressed as soon as possible. The OPA database change process can be accessed at the following link: https://opanet.hrsa.gov/OPA/CRPublicSearch.aspx Once completed, the existing or new Authorizing official will receive an email from HRSA asking to confirm any updates.
