  5. Federal EIN (Employer Identification Number provided by the IRS)

  6. Company Address (official business location)

  7. Date of First Operations (the date your business officially started operations)

  8. Business Contact Person Name (the primary contact person for business inquiries)

  9. SSN (Social Security Number of the responsible individual; usually needed for sole proprietors or business owners)

  10. Phone Number (best contact number for your business)
