Can someone else talk with Louisiana Blue on my behalf about my information?

You can authorize another person or an organization to speak to us about your claims, benefits, and other private health information. Whoever you choose to represent you is your authorized delegate.

Additional things to know:

  • Authorized delegates can only receive information about the member. They cannot make policy changes unless the member also gives them power of attorney (POA).

  • If the member is a minor child, authorization will end on that child’s 18th birthday.

  • For members 18 and older, authorization will be valid as long as the member has active coverage with Louisiana Blue.

To assign an authorized delegate to speak to us on your behalf, complete the Authorized Delegate form and follow instructions on the form to submit.
