There are several ways to obtain your ID card:
Download Digital ID Card
You can download a digital version of your ID card to use if you lost your physical ID card or haven’t received your cards in the mail yet.
Log in to MyLABlue on your computer or mobile app. If you need to set up an account, view How do I sign up for MyLABlue?
At top left, click the hamburger menu.
In list, under My Plans, select ID Cards. The front and back image of your member ID card displays with the same information as your physical ID card.
Email Request
Request your cards by filling out our secure Member Inquiry Form.
Call Customer Service
Call 1-800-495-2583 to speak to a customer service representative to request new ID cards be mailed to you.
