Claims information is considered personal health information (PHI) and is governed by federal regulations under the Health Insurance Portability and Accountability Act (HIPAA).
These regulations, along with state law and Louisiana Blue company policy, dictate that members age 18 and older have access to only their own PHI unless they authorize this information to be shared with people they designate. To see claims information for Louisiana Blue members age 18 and older, they must designate you as a proxy on their MyLABlue account.
To request full proxy access, you must set up a MyLABlue account and follow these steps:
Login to your MyLABlue account.
At top left, click the hamburger menu.
In list, under My Record, click Questionnaires. The Questionnaires page displays.
Click Proxy Access Request Form.
Next to “Are you requesting access to someone else’s account,” click Yes.
Complete the fields and upload appropriate legal documentation.
Click Continue. A screen displays to submit.
If needed, click Responses to view your responses.
Click Submit.
