After I submitted a claim, why did I receive a letter asking me to submit a new claim?

You may have received this letter because your claim did not include all the information needed for processing.

You should submit a new claim form with corrections indicated in the letter. You can also call the Customer Service number on the back of your ID card to find out what additional information, if any, is needed for processing.
