How do I change my last name due to a marriage or divorce?

This is how you change your name based on how you bought your policy:

Policy Bought Through the Health Insurance Marketplace

Call the Marketplace 1-800-318-2596.

Policy Bought Through a Group

Contact your employer.

Policy Bought Through Louisiana Blue

  • Download and print the Change of Status Card.
    OR

  • Submit a written request that includes:

    • Member ID number

    • Member name

    • Indication you are requesting a name change

    • Current date (date of request)

    • Subscriber signature

Submit completed Change of Status Card or written request along with copy of legal documentation (such as driver's license, marriage license, or Social Security card) via one of the following methods:
