How do I find my tax statements?

An IRS 1095 tax form provides proof of insurance and includes information that Louisiana Blue members must report on their tax returns.

The IRS requires 1095 tax forms to be mailed or made available to policyholders by the following dates (or the next business day if date falls on a non-business day):

  • 1095-A form - January 31

  • 1095-B and 1095-C form - March 2

If you have questions, do the following based on how you bought your policy:

  • Health Insurance Marketplace - Call the Marketplace at 1-800-318-2596.

  • Louisiana Blue - Call the number on the back of your ID card to speak to a customer service representative.
