Cancel your policy based on how you bought it and the reason for cancellation:
Through the Health Insurance Marketplace - Call the Marketplace 1-800-318-2596.
Through a Group - Contact your employer.
Through Louisiana Blue - Download and print the Change of Status Card and submit via one of the following methods:

Policy Cancelled Due to Death of Policyholder
To cancel policy and obtain new coverage for any remaining members, do the following based on how the policy was purchased:
Through the Health Insurance Marketplace- Call the Marketplace 1-800-318-2596.
Through Louisiana Blue- Submit completed Individual Plan Cancellation Form - Death of Policyholder. If remaining members need coverage, call a Louisiana Blue preferred agent 1-800-687-4672.
Important Considerations
Policies will cancel on the 1st or 15th, depending on when we receive the request:
Before the 15th- The policy will cancel on the 15th.
Between the 15th and end of the month- The policy will cancel on the 1st of the following month.
Refunds must be specifically requested, even if cancelling immediately. You may not be refunded the full amount you paid. Louisiana Blue determines the amount and mails it to the address on file.
Auto drafts already in process might not be cancelled until the next billing cycle.
Auto drafts must be cancelled the same way they were set up:
Through Bill Pay at Financial Institution - Contact your financial institution or bank. Louisiana Blue cannot cancel these payments.
Through Louisiana Blue - Choose one of the following options to cancel auto draft:
Cancel in MyLABlue
Set up a MyLABlue account and follow these steps:
Log in to your MyLABlue account.
At top left, click the hamburger menu.
In list, under Financial Tools, click Pay My Bill. A screen displays that says "Pay My Bill."
Click the Pay My Bill icon. A screen with your payment information displays.
Click Make a Payment.
Select Payment Methods tab.
In Recurring Payments section, next to the auto draft you want to cancel, click Remove.

Call Louisiana Blue- Call the number on the back of your ID card to speak to a customer service representative.
