Please view the following ways to a policy and select the choice that meets your needs.

Individual Blue Cross and HMOLA

A written request to cancel the policy must be submitted to Blue Cross and Blue Shield of Louisiana. The request must be a statement that includes:

  • Policy Number/Member ID

  • Member’s Name

  • Cancellation date

  • Current date (date of request)

  • Subscriber’s signature.

  • If the policy was set up on bank draft using the BCBSLA bank draft form, please request to cancel the bank draft along with your written request.

Note:

  1. If you set up bank draft or recurring credit cards payments in My Account, your bank draft or recurring payment must be canceled in My Account.

  2. Please contact your Financial Institution for recurring payments set up through Bill Pay.

Healthcare Federal Exchange Policy

Policies purchased on the Healthcare Federal Exchange:

  • Call the Federal Marketplace at 1-800-318-2596 to cancel a policy.

  • If you set up a bank draft, call the number on the back of your BCBSLA ID card. A Customer Service Advisors can assist you with cancelling your bank draft.

Note:

  1. If you set up bank draft or recurring credit cards payments in My Account, your bank draft or recurring payment must be canceled in My Account.

  2. Please contact your Financial Institution for recurring payments set up through Bill Pay.


Group Policy

Purchased through your employer:

  • Call your employer Group Leader or Human Resource Department to cancel the policy.


Deceased Policyholder

If the subscriber has one or more dependents on the policy, Please contact:

  • Federal Exchange Marketplace within 30 days following the subscriber’s death to obtain a policy at 1-800-318-2596

  • BCBS individual and group policies contact our Blue Cross preferred agents at 1-800-687-4672 for available coverage options.

Policies are cancelled on the 1st and the 15th of the month.

  • If you are requesting to cancel your policy on the 1st of the month, your request must be received prior to the 1st of the month that you are requesting cancellation; otherwise, your policy will be canceled on the 15th of that month.

  • If you are requesting to cancel your policy on the 15th of that month, your request must be received prior to 15th of that month.

If your premium payment for the cancellation month was received prior to the cancellation date, a refund will be mailed to the address on file.
