Where to find these examples
When you create a new Matter intake form, the following examples are pre-registered as templates on the [Matter intake form] settings screen. You can use them as-is or modify them as needed.
1. Example: Non-Disclosure Agreement (NDA)

Form Description Example:
Use this form to request the preparation or review of a non-disclosure agreement.

Example Purpose:
Use this form to request the creation or review of a sales agreement (including individual or master agreements).

Which party’s template is used
Due Date and Urgency
Seller party / Buyer party
Details of goods/services, delivery method, inspection period, payment terms
Risks or points to share with Legal
3. Example: Service Agreement (With Deliverables)
Form Name Example:
Service Agreement with Deliverables Request Form
Notes:
This applies when the agreement includes deliverables, e.g., design, videos, reports.
Key Fields:
Parties involved, template used, due date, urgency
Scope of work, deliverables, delivery method/location, delivery deadline
Ownership of deliverables, compensation, payment due date
Risks or concerns to share with Legal
4. Example: Service Agreement (Without Deliverables)

Notes:
For services like consulting, maintenance, or advisory work that do not result in deliverables.
Key Fields:
Scope of work, compensation, payment schedule
Contract term, risks or points to share with Legal
5. Example: General Contract

Roles of your company and counterparty

Submit general legal questions or advice requests.
Key Fields:
Subject of consultation, background, specific questions
Due date and urgency
Related files or materials
Additional Tips
The [Settings] Tab for each form should specify the Matter Type, e.g., Contract Review or Legal Consultation.
To create your own form from scratch, use [+ Add Field] → [Free Text] for any custom questions.
If you use these examples, adapt them to your company's actual legal workflows and preferred level of detail.
