Creating a Form in Lever360

Create and manage forms in Lever360 to collect customer signatures for authorizations and approvals via email or mobile signing.

1. Accessing the Form Builder

  1. Navigate to the Main Menu.

  2. Scroll down and click the Settings icon (gear icon).

  3. In the Company Settings menu, select Form Templates.

2. Creating a New Form

  • Click "Create Form Template".

  • Enter a Title and Description for the form.

    • The Title is the primary identifier.

    • The Description provides additional context and will appear when sending the form.

3. Formatting and Adding Job Data (Keys)

  • The Form Builder allows rich text formatting with:

    • Bold, Italics, Underline, Strikethrough

    • Lists: Bullet points, numbered lists, checklists

    • Tables: Used to structure fields like customer and insurance details

    • Images: Upload company logos or other visuals

    • Columns: Use tables to separate content neatly

  • Inserting Dynamic Fields (Job Keys):

    • Click the Insert Job Key dropdown.

    • Select fields such as Job Name, Customer Name, Address, Insurance Company, etc.

    • The system will automatically populate these fields when the form is sent.

4. Adding Signature Fields

For the form to be signable, you must insert a Signature Field:

  1. Click the Insert Signature Key dropdown.

  2. Select Signature to place a signature field in the form.

  3. (Optional) Add Signature Date to auto-stamp the signing date.

💡 Tip: Formatting elements like bolding job fields (e.g., Job Name) help key information stand out when sent to customers.

5. Saving & Managing Forms

  • After creating your form, make sure to click Save Changes.

  • To preview the form before sending, click Preview Form.

  • You can edit existing forms from the Form Templates list.

  • Ensure that forms are correctly formatted before sending.


Getting the Form Signed
