Yes! You can create a survey or questionnaire on your Linktree using a Contact Form.

How to add a contact form to your Linktree

In My Linktree, click Edit to open the Links tab.
Click +Add and Search for "Form". Select it from the results.
From the template options, select Create custom form and click Add.
In the Info section you can Edit form details.
To get started click + Add field.
Choose from field Suggestions or Write your own and check Required for mandatory questions.
Add an Intro (optional) and an Outro message.
(Optional) Toggle on Custom T&Cs to link to additional terms. (Available on Pro and Premium plans)
(Optional) Ask visitors to join your mailing list when they submit the form with Mailing list opt-in.
- 📝Note: you must add an email or phone field to collect mailing list sign-ups

Where do survey responses go?

All responses are stored in the Audience tab in your Linktree admin.
