Yes! You can create a survey or questionnaire on your Linktree using a Contact Form. This lets you collect responses directly from your visitors by adding custom questions to your form.

How to add a contact form to your Linktree


  1. In your Linktree Admin, go to the My Linktree page.

    • 📱Using your mobile device? Tap Edit first.

  2. Hit the + Add button.

  3. In the URL or App search box, search for "Contact form" and click Add.

  4. Give your form a Title (e.g., "We’d love your feedback!" or "Tell us what you think").

  5. Choose Blank Form to create a custom survey.

  6. Tap Add question to customize your survey by adding multiple questions.

  7. Use the Label field to specify the question you want to ask (e.g., "How satisfied are you with our service?").

  8. Choose a Field Type for each question (e.g., text box, dropdown, multiple-choice).

    • 📝 Note: Some field types will have additional options for you to complete.

  9. Tap Save after adding your questions.

  10. Check the Required box next to each question you'd like to be mandatory.

  11. Tap Save after adding your questions.

  12. Add an optional Introduction at the top of your form to greet visitors.

  13. Personalize the Thank You Message to show appreciation after submission.

  14. Customize your form by adding an Introduction and a Thank You Message.

  15. (Optional) Toggle on Custom T&Cs if you need to link to additional terms and conditions. (Available on Pro and Premium plans)

  16. Tap Done to finish setting up your form.

Where do survey responses go?


All responses to your survey or questionnaire will be stored in the Audience tab within your Linktree Admin. Audience is the home for all submissions, including other Audience tools like Subscribe or Digital Downloads.
