Yes! You can create a survey or questionnaire on your Linktree using a Contact Form.

How to add a contact form to your Linktree

In My Linktree, go to the Links page.
Hit the + Add button.
Search for "Contact form" and click Add.
Give your form a Title.
Choose Blank Form to create a custom survey.
Tap Add question to add your survey questions.
Use the Label field to specify the question, choose a Field Type, and check Required for mandatory questions.
Tap Save after adding your questions.
Add an optional Introduction and a Thank You Message.
(Optional) Toggle on Custom T&Cs to link to additional terms. (Available on Pro and Premium plans)
Tap Done.

Where do survey responses go?

All responses are stored in the Audience tab in your Linktree admin.
