To participate in the study, all participants are required to provide informed consent. This is done by completing the eICF, or electronic informed consent form.

Follow the steps below to upload the form.

Steps

1. Complete the pre-screening process. The next screen will give you some information on eICF and ask you to click Next to proceed.

2. The following screens will give you all the information you need to submit your consent form.

a. Part I contains study information

b. Part II contains information on how your data will be processed

c. Part III is a glossary to help you understand what you have read

d. Read Part IV of the consent form. Once you have read through all the information thoroughly, you can choose to participate in the study, or leave the study. If you choose not to participate in the study, you can leave the process immediately or contact the study team to discuss this further. If you choose to participate, go to step 3.

3. Select whether you would like your data to be used in future studies.

4. Next, select if you allow the team retrieve your information from the vaccine registry.

5. Enter your password. This is the password you used to register for the app.

6. Enter your first and last name & put your signature in the box provided.

7. Enter your name again.
