Getting Started with Feedback Surveys

Learn how to create and test your first feedback survey in Mouseflow

Feedback Surveys in Mouseflow let you collect direct input from your visitors while they browse your site. This article walks you through creating a survey, choosing question types, setting logic, and testing your setup. By the end, you’ll have a ready-to-use survey that you can preview and test.


What Are Feedback Surveys?

Feedback Surveys appear as small widgets on your site that ask visitors questions in real time. They are useful for:

  • Collecting opinions about design or usability

  • Measuring satisfaction (e.g., NPS)

  • Understanding why users drop off or convert

Important note: Surveys require that Mouseflow is recording a session. Displaying a survey does not consume extra credits.


Create a New Survey

  1. Navigate to the Feedback section in your Mouseflow project.

  2. Click Add New Survey.

  3. Give your survey a name at the top of the editor. This name is for your reference only. Visitors will not see it.


Add Questions

You can add up to 100 questions per survey. There are 6 different question types and the ability to add a welcome message:

  • Welcome Message: A short greeting that appears before the survey begins.

  • Multiselect (Checkboxes): Let visitors select multiple answers.

  • Single Select (Radio Buttons): Visitors choose one option.

  • Free Text (Small): A short written response.

  • Free Text (Large): A longer written response.

  • Rating Scale: Numeric scale you define (e.g., 1–5 stars).

  • Net Promoter Score (NPS®): This question type is a widely used market research method that can measure answers to your question using numbers or smiley faces. To learn more about NPS®, check out this article.

Privacy Disclaimer: Per our Terms of Use you may not request personal data from your website visitors in feedback surveys. Only anonymous information may be collected from website visitors. Please refer to this article for details on the types of data that must not be collected using feedback surveys, which may vary depending on applicable local regulations.

Question Options

Each question has flexible settings:

  • Required Answer → make the question mandatory.

  • Randomize Answers → shuffle answer order to avoid bias.

  • Keep “Other” Last → pin one option at the bottom while others shuffle.

  • Free Text Confirmation → add a checkbox that must be ticked before submitting.

Add Logic to your questions

Logic allows you to guide visitors through your survey based on their answers.

  • Works with Radio Buttons, Rating Scale, and NPS® questions.

  • Example: If a visitor gives a low rating, send them to a follow-up question asking why.


Success Step

After the survey ends, show a final message (thank you, follow-up link, etc.).

  • You can add links with [TEXT](URL).

Preview Your Survey

The right-hand preview updates as you build your survey.

  • Click through it to test the experience.

  • Every change you make appears instantly.
