Forms are pre-configured digital documents that are associated with an incident type to allow responders to report relevant information from the field to the dispatcher and vice versa. You can attach one or more form templates to each incident type. These forms appear in the Dispatcher and in the mobile application when an incident of that type is launched.
The procedures described in this article are a continuation of the procedures described in Creating and Editing Form Templates

Set Field as From Title

You can set a Label or a Text Box field as the form title, meaning that when you open a new incident, the form title takes on the name of the Label field or the value entered in the Text Box field. To set a field as the form title, select Dynamic form title ? in the advanced settings area on the left of the page.

Note: You can only set one field as a Dynamic Form Title, and then only if the field is a Text Box or a Label type field.

Visible Field

You can define one field to be visible or invisible based on the content of other fields in the form.
Note: If you select a field to be a Visible field, you cannot also select it to be a Required field, and vice versa.

To set a visible field:

  1. Select the field you want to be a visible field.
  2. In the Basic Settings area, select Visible Field.
  3. Click Preconditions.
  4. In the Visible Field window, define the conditions under which the field will be visible. There is no limit to the number of conditions you define.
  5. Click Save to save your settings.

Note: The Visible field option is only relevant in forms that are completed by the dispatcher.

Required Field in Dispatcher

You can define one field to be required field based on the content of other fields.
Note: If you select a field to be a Required field, you cannot also select it to be a Visible field, and vice versa.

To set a required field:

  1. Select the field you want to be a required field.
  2. In the Basic Settings area, select Required field in Dispatcher.
  3. Click Preconditions.
  4. In the Required field Visible Fields window, define the conditions under which the field will be a required field in the Dispatcher. There is no limit to the number of conditions you define.
  5. Click Save to save your settings.

Assigning a Form to Incident Types

After creating a form template you can assign it to an Incident. You can do this either from the Form tab or from the Incidents panel.
To assign a form template to an incident from the Form tab:

  1. Click Assign to view the list of incidents.

2. In the Assign to Incident Type dropdown list, select the incidents to which you want to assign the form.

3. Click the X in the top right corner of the Assign to Incident Type dropdown list.

Saving the Form Template

After making you changes, click Save to save the form template.
