Create your own risk assessment templates

You can create your own custom risk assessment templates using the Template function

Currently, Nursebuddy offers you 3 templates to use for Risk Assessments forms:

  • Action, Benefit, Loses

  • HSE Risk Assessment

  • Risk to others

Each of these templates contain a series of pre-defined questions but there may be times where these templates don't adjust to what your risk assessments are.

In those cases, you can design and then create your own templates to be used for different types of risk assessments.

To start creating your own templates follow these steps:

1. Draft a Risk Assessment Template:

  • Create a draft of the risk assessment template in a Google Sheet or Excel, outlining the structure and questions you want to include.

  • Fields like the service user name, assessment date, completed by are not necessary since these are included automatically by the system in each Risk Assessment form at client's level (in the client's profile).

2. Create the template in Nursebuddy:

  • Go to Settings > Clients > Risk Assessments.

  • Click on the "Templates" tab to access the function for creating templates.

  • Click on the "Add Template" button.

  • Give the template a unique and descriptive name (e.g., Risk Assessment Template V1).

  • Add fields to the template based on your draft, using options like radio buttons for yes/no responses or notes for additional details.

  • You can use the record value option to indicate ratings (e.g., low, medium, high) for each question, just make sure the title of each field is unique. 

  • Ensure the template is structured according to your draft with sequential numbering for questions.

    Example 1: A simple template using the Note action with numbers from 1 to 5

    Example 2: A template with a Note box + Radio button (Select one)

Don't add client-specific information in this step!!

By default the system will add for you in each form the following items:

  • Rating (based on a R.A.G system)

  • Carried by

  • Assessment date

  • Next review (it will generate a notification in the notification centre 7 days before the review date entered here)

 

3. Save the Template:

  • Review the template for accuracy and completeness.

  • Click "Save" to finalise the template.

  • Note that once saved, templates cannot be edited, so ensure that all details are correct before saving.

4. Utilise the Template:

  • In the tab Forms, when clicking +Add Form, select the template you created to add forms for specific risks by selecting it from the list of available templates, e.g: "Fire Hazard Risk Assessment"

  • Pre-fill in the blank spaces with relevant questions for that specific risk assessment.

5. Fill out the form with each client's information

When you have your form ready and saved in Settings, go to your client's profile and in the Risk Assessment tab proceed to edit the risk assessment or start creating a new one using the different forms you have in the drop down list

In this step is where you will add the specific information of your clients based on the risk you have assessed.


Cautionary Notes:

  • Templates cannot be edited after saving, so ensure accuracy before finalising.

  • Template names cannot be duplicated to prevent confusion in selection, make sure to create a standard name with number of versions e.g "Risk Assessment v.1"

  • Ensure all necessary fields and question types are included in the template for comprehensive risk assessment.

  • The name of the fields in the templates cannot be repeated, so you can use 1, 2, 3 4, etc or a, b, c, d in each field and then in the form pre fill the questions.

Tips for Efficiency:

  • Preparing a draft of the template structure beforehand helps streamline the creation process.

  • Use consistent numbering and labelling for questions to maintain clarity and organisation.

  • Utilise radio buttons for simple yes/no responses and notes for additional details to enhance user experience.
