Acceptable Documentation for Utah Fits All (UFA) Reimbursements

Learn what receipts, invoices, and proof of payment are required for Utah Fits All (UFA) reimbursement requests, including payment app requirements and documents that are not accepted.

If you're participating in the Utah Fits All (UFA) Program and are eligible to submit reimbursement requests, you must provide documentation showing what you purchased and that payment was completed.

Submitting complete, accurate documentation helps prevent delays in reviewing your reimbursement request.

What Documentation Is Required?

Your documentation must show:

  • What items or services were purchased

  • The date of purchase

  • The business or service provider

  • The amount paid

  • The payment method used

  • Confirmation that payment was completed

⚠️Important: You can upload only one file per reimbursement request. If you have multiple documents, such as an invoice and proof of payment, combine them into a single PDF or image before uploading.

Receipt Requirements

An acceptable receipt must include all six details:

  1. Vendor information: The name of the business, store, or service provider.

  2. Transaction date: The date of purchase, which must match the expense date entered in Odyssey.

  3. Itemized purchases: A list of each item or service with its individual price.

  4. Total amount: The full amount paid, including applicable taxes and fees.

  5. Payment method: How the purchase was paid for. Purchases paid entirely with gift cards or rewards points are not eligible.

  6. Proof of payment: Confirmation that payment was completed, such as "Paid," "Zero Balance," or payment details on the receipt.

Cash and Check Payments

Purchases made with cash or check must have documentation generated by the vendor's point-of-sale (POS) system.

Handwritten receipts, manually typed receipts, and self-prepared invoices are not accepted as documentation for cash or check purchases.

Receipts Created Using Word Processors or Document Editors

Receipts created in programs such as word processors or document editors, rather than generated by a POS system, require additional proof of payment.

These receipts may lack system-generated information such as:

  • Transaction IDs

  • Terminal numbers

  • Approval codes

  • Automatic timestamps

If your receipt was created using a word processor or document editor, you must also provide one of the following:

  • A bank or credit card statement showing the vendor's name, transaction amount, and date

  • A digital payment confirmation screenshot

Payments Made Through Venmo, PayPal, Zelle, or Cash App

If you paid through Venmo, PayPal, Zelle, Cash App, or a similar payment service, you must provide both a receipt or invoice and proof of the completed payment.

Your payment confirmation screenshot must show:

  • Payment status: Confirmation that the payment was completed

  • Date and time: When the payment occurred

  • Transaction ID: The transaction or confirmation number

  • Recipient information: The name or identifying information of the vendor or service provider

⚠️Important: A payment app screenshot alone is not sufficient. You must also provide an itemized receipt or invoice showing what was purchased.

Remember to combine both documents into a single file before uploading.

What's the Difference Between a Receipt and an Invoice?

Receipt

A receipt confirms that a purchase was paid for. It serves as proof of purchase and can be used for reimbursement when it includes all required information.

Invoice

An invoice lists goods or services and requests payment. It does not, by itself, confirm that payment was completed.

If you submit an invoice, you must also provide proof of payment.

Documentation That Is Not Accepted

Not Accepted on Its Own

The following documents cannot be submitted without additional supporting documentation:

  • Payment app screenshots

  • Bank statements

  • Credit card statements

  • Invoices

  • Bills

  • Order confirmations that do not show completed payment

These documents may be used as supporting proof of payment when accompanied by an acceptable itemized receipt or invoice.

Never Accepted

The following documents are not accepted as reimbursement documentation:

  • Handwritten receipts

  • Non-itemized receipts

  • Photos of non-itemized receipts

  • Pictures or copies of checks

Tips to Avoid Reimbursement Delays

Before submitting your reimbursement request:

  • Ask for an itemized receipt when making a purchase.

  • Check that the receipt includes all required information.

  • Make sure your payment has been completed.

  • Combine supporting documents into one PDF or image if needed.

  • Take clear, readable photos of paper receipts.

  • Keep your receipts until your reimbursement is approved.

  • Review your uploaded file before submitting your request.
