Create a Team Registration Form

Use the Team Registration Form to gather any additional information needed for seeding, season planning, or scheduling.

  1. In Programs, under the Team Reg heading, select Team Reg Form.

2. To begin building out the form, click on the mini menu and select Add Item.

3. Next, you will be prompted to choose your Input Item.

Input/Layout Items Overview

  • Text Input: Open-ended input where a registrant can answer as necessary

  • Multiple Choice: Limit the number of possible answers in a multiple-choice format. Select a display option and input answer options that can be moved and made the default.

  • Checkbox: Used for simple either/or questions where a registrant can check the box to answer.

  • Memo Text: For longer text inputs required.

  • Free-form Text: Use this layout item to add any instructions or context to the registration form.

  • Visual Spacer: Horizontal line that can be used to create space between registration fields within the same section.

4. Once you have chosen your Input Item, enter the item label, data name, or response options, etc. Input item options will vary depending on the item you selected.

5. Once you have saved the Item Item it will display on the form. From there, you can hover over the item to move, edit, or delete the Input.

6. Another option to add content on the Form is to select Add Section. You'll be given two options:

  • Blank Section (no section items): This will generate a blank section where you can add form contents manually.

  • Templated Sections & Items: This option will display a dropdown window where you can automatically add a templated form you have created in the Settings > Forms & Docs > Web Forms area.

7. As you build out the form, use the mini menu in each section to add new items, edit the section Title, or delete the section.

8. As you build the form, you can use the Undo and Redo options to easily make and manage edits.

9. Use the Public View button to preview how the form will display to registrants during the Team Registration process.

Tip: If you'd like to reuse this form for future seasons, click the mini menu and select Save as Template. This will save the form and all of its contents as a template that can be quickly applied to other seasons later on.
