When adding a maternity/paternity leave, you will need to make sure that:
All fields needs to be completed, like Due Date, Actual Start Date and Actual End Date.
There are no multiple maternity entries for that specific user.
You can also encounter cases when public holidays are being deducted from entitlement while on maternity leave.
