Create or edit Fast Feedback form templates

Learn how to configure and customise Fast Feedback templates in Performance Leader. Step-by-step guide covering form creation, editing, reordering, and management for professional services firms.

Fast Feedback templates are the bones of your feedback culture—they shape how your people give and request meaningful feedback. This guide walks you through everything you need to configure, customise and manage these templates with confidence.

We'll cover how to reorder forms, create new templates from scratch, edit existing ones, and explore the additional tools available to you. By the end, you'll be able to tailor your Fast Feedback experience to perfectly match your firm's needs. Watch the video walkthrough below or follow the step-by-step instructions for each key function.

How do give and request feedback forms work together?

In Performance Leader, give and request feedback are two sides of the same Fast Feedback template. When someone requests feedback using a template, the person they've requested it from sees the context provided and responds using the corresponding "give" form. However, both forms can also work independently—you don't need to wait for a request to give feedback, and vice versa.

Example: The After Action Review template has four prompts for giving feedback and one prompt for requesting it. These are connected but can be used separately.

How do I access and manage my Fast Feedback templates?

All template configuration happens in one central location. To find it:

  1. Navigate to Manage apps from your admin menu

  2. Select Fast Feedback

  3. Click into the Configuration and tools section

  4. You'll see all your templates listed here, with options to reorder, edit, or create new ones

How do I change the order of my feedback forms?

The order of forms in your configuration determines how they appear to users when they're giving or requesting feedback. Reordering is simple:

  1. In the Configuration and tools section, locate the form you want to move

  2. Click and drag the form to your desired position

  3. Refresh the give or request feedback page to see your changes reflected

Note: The order is the same for both give and request forms, so changing one affects both.

How do I create a new Fast Feedback template?

Creating a new template allows you to design feedback forms that match specific scenarios or needs in your firm.

  1. Click Add template in the Configuration & tools section to open the Create Fast Feedback Template dialog

  2. On the Settings tab, enter a Template Name and Description (the description is visible to users)

  3. Set the template options: - Timeline Display: tick Display request in timeline to show feedback requests in the recipient's timeline - Feedback Privacy: tick Feedback is private if responses should only be visible to supervisors, not the recipient - Enabled: leave this on to make the template selectable (disabled templates can't be chosen when creating a request)

  4. Use the Request form and Feedback form tabs to build your questions (see next section)

  5. Click Create Template

How do I add questions to my feedback forms?

Once you've created your template, you'll need to add the questions that users will respond to. You have flexibility in question types:

  1. Open the Request form tab (fields employees complete when requesting) or the Feedback form tab (fields providers complete when giving)

  2. Select your question type. Some options include: - Text area (for longer responses) - Dropdown (with custom options) - Date field - Radio buttons - Checkboxes - Ratings

  3. Type in your question text

  4. Add optional instructions to provide context or guidance

  5. Mark the question as required or optional

  6. Click Save

Tip: You can reorder questions by using the up and down arrows next to each field.

How do I edit an existing Fast Feedback template?

Need to refine a template that's already in use? Here's how to make changes:

  1. From the Configuration and tools section, locate the template you want to edit

  2. Click the edit icon (pencil) next to the template name

  3. Make your changes to: - Template name or description - Template settings (display, privacy, availability) - Question text, order, or requirements - Optional instruction text

  4. Important: After making changes, click Update template to save

  5. Refresh the give or request feedback page to see your updates

Common mistake: Forgetting to click "Update template" means your changes won't be saved. Always check you've clicked update before leaving the page.
